How is a driver’s car set up determined?

Much like a team in any other international series. Before each event F2's Chief Engineer establishes a recommended start 'baseline' set up and this is provided to the driver. The driver can then choose to run with this, or, in consultation with their engineer, modify it to start the event. During the race weekend the driver will liaise with their engineer and evolve their set up as mutually decided.
